The number of people affected is in the double-digit percentage range, the ratio of men to women is about 5 to 1, daytime fatigue and atten\u00adtion deficit are the obvious effects of the disease. If untreated, it can also lead to hidden compli\u00adca\u00adtions such as high blood pres\u00adsure, cardiac insuf\u00adfi\u00adciency (dimin\u00adished cardiac output), heart rhythm distur\u00adbances or myocar\u00addial infarc\u00adtion. A proven form of therapy supports the body\u2019s own respi\u00adra\u00adtory reflexes through the controlled injec\u00adtion of air into the lungs.&nbsp;<\/p>\n<p>A new, highly dynamic centrifugal fan now supports breathing through the controlled supply of fresh air. The light\u00adweight, effi\u00adcient small fan is also well suited for long battery oper\u00ada\u00adtion, for example for a mobile sleep therapy device. Partic\u00adu\u00adlarly when using apnea machines at home or as a venti\u00adlator in the inten\u00adsive care unit, simple oper\u00ada\u00adtion is also impor\u00adtant. If, for example, a venti\u00adlator is used during sleep, such a device must be located close to the user and must not inter\u00adfere with healthy sleep due to oper\u00adating noises.&nbsp; In order to meet these require\u00adments, the fan and drive specialist ebm-papst from St. Georgen in the Black Forest devel\u00adoped the RV45 centrifugal fan for respi\u00adra\u00adtory protec\u00adtion devices and similar dynamic appli\u00adca\u00adtions that meets these require\u00adments \u2013 safe, reli\u00adable, effi\u00adcient and quiet.<\/p>\n<h2>How does mechan\u00adical venti\u00adlation work?<\/h2>\n<div class=\"box box-right\">\n<h2 class=\"RBSB-Zwischen\"><span lang=\"EN-US\" style=\"font-family: 'Arial Narrow',sans-serif;\">Sleep apnea: <br>\nWhat is it? <\/span><\/h2>\n<p class=\"RBSText\"><span lang=\"EN-US\" style=\"font-family: 'Arial Narrow',sans-serif;\">About 10 % of all sleepers over 40 years of age are affected by sleep apnea world\u00adwide, i.e. tempo\u00adrary respi\u00adra\u00adtory arrests that occur during sleep. Sleep apnea syndrome is likely to occur if there are more than ten respi\u00adra\u00adtory arrests per hour that last longer than 10 seconds. The periods of breathing cessa\u00adtion occur up to twenty times an hour in severe cases. The breathing pause is regis\u00adtered in the sleeper\u2019s brain and over\u00adcome by an awak\u00adening reac\u00adtion. <\/span><\/p>\n<p class=\"RBSText\"><span lang=\"EN-US\" style=\"font-family: 'Arial Narrow',sans-serif;\">This wake-up reac\u00adtion is vital and prevents suffo\u00adca\u00adtion but disturbs sleep. The sleeper changes from a deep sleep stage to a super\u00adfi\u00adcial sleep. Snorers in partic\u00adular are affected by this disease. Due to sleep distur\u00adbances and lack of oxygen supply during the respi\u00adra\u00adtory arrest phases, the patient feels unrested and tired in the morning, often with daytime sleepi\u00adness and a lack of atten\u00adtion. <\/span><\/p>\n<p class=\"RBSText\"><span lang=\"EN-US\" style=\"font-family: 'Arial Narrow',sans-serif;\">In addi\u00adtion, chronic untreated apnea syndrome can often lead to high blood pres\u00adsure, heart failure (dimin\u00adished cardiac output), cardiac arrhyth\u00admias and an increased tendency to heart attacks and strokes.<\/span><\/p>\n<\/div>\n<p>The tech\u00adnology behind it is easy to under\u00adstand: Air pres\u00adsure gener\u00adated by the fan inflates the lungs and thus supports breathing. But the trick is in the details: The appli\u00adca\u00adtion requires highly dynamic fan oper\u00ada\u00adtion, meaning very rapid changes in speed. However, smooth running in dynamic oper\u00ada\u00adtion is not easy to imple\u00adment. The biolog\u00adical prop\u00ader\u00adties of breathing demand swift and smooth control of the air flow and pres\u00adsure. For example, it is often neces\u00adsary to generate a high \u201cblowing pres\u00adsure\u201d for a short time at the begin\u00adning of the inhala\u00adtion sequence. This pres\u00adsure raises the flaccid soft palate and allows air to enter the wind\u00adpipe. However, this \u201cinitial high air pres\u00adsure\u201d may only be applied very briefly and it must rise quickly, but not abruptly, in order to give the palate the neces\u00adsary time to clear the wind\u00adpipe.<\/p>\n<p>After opening the soft palate, the pres\u00adsure must be quickly reduced to the inhala\u00adtion level prescribed by the doctor and sustained at this level. To match the patient\u2019s respi\u00adra\u00adtion, the supporting flow rate and pres\u00adsure must be constantly adjusted. In order to exhale, the pres\u00adsure must then drop quickly again, but not abruptly, in order to allow unhin\u00addered exha\u00adla\u00adtion. For the breathing dynamics, approx\u00adi\u00admately 200 ms control time is typical with volume flows around 150 l\/min and pres\u00adsure fluc\u00adtu\u00ada\u00adtions from 400 Pa to 2,000 Pa. The pres\u00adsure should be kept as low as possible and never exceed 3,500 Pa to avoid lung damage.<\/p>\n<p>On the other hand, the adjustable high volume flow rate is impor\u00adtant in order to be able to reli\u00adably compen\u00adsate for leaks, e.g. in the respi\u00adra\u00adtory mask due to poor fit or in the case of bearded wearers. In order to regu\u00adlate the neces\u00adsary vari\u00ada\u00adtion in volume flow and pres\u00adsure in prac\u00adtice, the speed of the centrifugal fan must there\u00adfore be quickly increased or decreased. The compact fan RV45, which has been opti\u00admized in terms of aero\u00addy\u00adnamics and motor dynamics, provides the neces\u00adsary air flow and drive tech\u00adnology. Despite the strongly fluc\u00adtu\u00adating oper\u00adating condi\u00adtions, the RV45 meets the absolute quiet\u00adness required so as not to disturb the sleep of the patient or anyone else sleeping nearby.<\/p>\n<h2>Tech\u00adnology in medical service<\/h2>\n<p>How can the different require\u00adments be trans\u00adlated into a product that can be used as univer\u00adsally as possible? The basic prereq\u00adui\u00adsite is the use of FDA-compliant mate\u00adrials for air-contacting parts that meet all rele\u00advant regu\u00adla\u00adtions world\u00adwide. At the same time, the entire aero\u00addy\u00adnamics of the fan was adapted to the appli\u00adca\u00adtion. Both at high and low revs, the air flow noise was signif\u00adi\u00adcantly reduced. The RV45 is designed for use in CPAP (Contin\u00aduous Posi\u00adtive Airway Pres\u00adsure) and for oper\u00ada\u00adtion with auto\u00admatic pres\u00adsure adap\u00adtion (APAP\/auto APA and BiLevel\/BIPAP). The motor, control elec\u00adtronics and the aero\u00addy\u00adnamics are designed in a way that each part supports the other in order to reach those diverse require\u00adments by synergy.<\/p>\n<blockquote>\n<p>Depending on the medical device, the user can provide their own specif\u00adi\u00adcally opti\u00admized control system.<\/p>\n<\/blockquote>\n<p>In order to equip the centrifugal blower opti\u00admally for the widest possible range of appli\u00adca\u00adtions, the Black Forest special\u00adists chose a highly dynamic, EC (elec\u00adtron\u00adi\u00adcally commu\u00adtated) internal rotor motor as the drive. The low moment of inertia of the rotor equipped with a powerful magnet accom\u00admo\u00addates the required dynamics. The magnet and coil design, was further opti\u00admized through exten\u00adsive simu\u00adla\u00adtion and testing. At the same time, the detent torque and struc\u00adture-borne noise exci\u00adta\u00adtion have been mini\u00admized and effi\u00adciency improved. Since only the bear\u00adings of the EC motor are subject to wear and tear, the use of main\u00adte\u00adnance-free ball bear\u00adings with special grease lubri\u00adca\u00adtion enabled the service life to be increased to 50,000 hours L<sub>10IPC<\/sub> (25 \u00b0C) after the stricter, in-house ebm-papst test condi\u00adtions were passed. (Image: ebm-papst)<\/p><\/div><\/div>\n<p>Because the delivery rate (linear) and the delivery pres\u00adsure (square) of a centrifugal fan impeller increase with the speed, the high speed of up to 50,000 rpm allows the fan to be very compact. The motor drive and control elec\u00adtronics are not included inside the RV45 blower and must be provided exter\u00adnally, which offers advan\u00adtages in matching the fan to the respec\u00adtive task. Depending on the medical device, the user can provide their own specif\u00adi\u00adcally opti\u00admized control system.<\/p>\n<p>However, for a wide range of stan\u00addard tasks or fast test oper\u00ada\u00adtion, a control module from the manu\u00adfac\u00adturer is avail\u00adable which is specially adapted to the motor. This \u201cPower Module RV45\u201d is suit\u00adable for simple speed control and includes a tachometer output, thus offering a plug &amp; play solu\u00adtion for the customer. This can be useful in both medical and indus\u00adtrial appli\u00adca\u00adtions, e.g. for the dynamic venti\u00adlation of fuel cells, air filter tech\u00adnology, pack\u00adaging machines, smoke detec\u00adtion systems, printed circuit board produc\u00adtion or exhaust air systems for soldering and welding gases as well as breathing appa\u00adratus and similar devices. (Graph: ebm-papst)<\/p><\/div><\/div>\n<p>The RV45 is very compact with only 64 x 69.5 x 54.5 mm and is avail\u00adable for oper\u00ada\u00adtion on two volt\u00adages 12 and 24 VDC. The power consump\u00adtion is around 43 W in free blowing oper\u00ada\u00adtion, but running in typical appli\u00adca\u00adtions with speed control can often average 20 W lower, which also ensures long running times in battery oper\u00ada\u00adtion. The possible high power consump\u00adtion is indis\u00adpens\u00adable for the short-term \u201csprint\u201d in order to fulfill the require\u00adments on high dynamic oper\u00ada\u00adtion. The maximum free blowing air flow is up to 410 l\/min and the maximum pres\u00adsure increase is over 5,000 Pa.<\/p>\n<p>This is suffi\u00adcient for large lung volumes, heavy soft palate cases or for use in secre\u00adtion mobi\u00adliza\u00adtion. All air-contacting parts of the 135&nbsp;g light-weight fan are constructed from FDA compliant mate\u00adrials which are harm\u00adless to human respi\u00adra\u00adtory phys\u00adi\u00adology. Thanks to the selected vibra\u00adtion damping mate\u00adrials and the opti\u00admized aero\u00addy\u00adnamic design, the oper\u00adating noise is mini\u00admized and is at a level similar to a whis\u00adpering conver\u00adsa\u00adtion.
